Documents & Information

After our telephone conversation and before you visit my office for your first appointment please print out and complete the health history form and bring a list of all of the medications that you currently take. If you don't have time to write your medications down, just bring them in a bag so we can sort out what you are taking.

I also need a copy of your most recent physical from your primary care doctor (it needs to be within the past two years). Additionally, a copy of any lab work that has been ordered.
